Charged But No Order Or Confirmation Email

I was charged two weeks ago for my order and neither got it nor received a confirmation order. Moreover, I emailed a week ago and after being told it would be looked into, I've received no follow up despite numerous attempts to be updated on the situation.

 

I checked out as a guest. I have no Order Number to provide. I'm hoping this can be taken care of immediately. Thanks.

I need the following info:

  1. Order number (example: 004343).
  2. Order date
  3. Name used
  4. shipping addy
  5. exact order
